Three steps to filing a complaint

How to file a complaint

Work through the three steps in order. The first two file the complaint with the state; the third tells the association, so we can track trends and advocate for Montana shops.

Get your customer to sign the Complaint Authorization form

The complaint carries far more weight when the vehicle owner signs it. Download the authorization, have your customer sign it, and keep a copy with your file — you will attach it to the state complaint in step 2.

Submit the complaint to the Montana Commissioner of Securities and Insurance

File online through the CSI website, or download the paper form and send it in by mail or email.
